Despite the efforts of governments and international aid agencies, investment in the quality and availability of health care services in developing countries is not working as it should. Although there have been improvements in many areas, their pace and spread have been uneven. In many cases the benefits of investment last only as long as funding continues. Dependence on external support is increasing, with little evidence of lasting change in local capacity to deal with the problems.
